Hypertension Case-Finding Service – Key Information and Support Resources

Core Reference Materials
The core documents underpinning the Hypertension Case-Finding Service are the Service Level Agreement (SLA) and the NICE Guideline NG136. These should be your first point of reference when delivering the service.

Upcoming Webinar – Service Changes from 1st October 2025
We will be hosting a further support webinar ahead of the upcoming service changes effective 1st October 2025. Details will be shared soon.

Pharmacy & Surgery Communications – Getting Started
Before beginning service delivery, we strongly recommend initiating a team-based discussion with your local practice manager and reception manager. Key points to cover:

  • Public health benefits of early hypertension detection
  • How the service complements primary care
  • Opportunities for referrals from surgeries for patients with known hypertension or long-term conditions

To support these conversations, use the BP Checks Summary document. It outlines relevant Quality and Outcomes Framework (QoF) targets of interest to practice teams and highlights the contractual and clinical benefits for both GP practices and pharmacies.
